Architectural Review

We will need the ARC Form along with the following information:

1. If an HOA we require a Site Plan, if a Condo we require a Floor Plan
2. Material Specifications
3. Permit (contact office to see what projects require permits, if any)
4. Vendor's Quote
5. Vendor's License
6. Vendor's Certificate of Insurance showing General Liability and Worker's Compensation Insurance (or Workers Compensation Exemption Letter) 
7. The following as the certificate holder and additional insured information required:
